If problems become more time-consuming than you can handle given the other demands in your life, the JFS care manager will step in to coordinate services, assist with legal, financial and medical issues, and give you peace of mind.
If you notice a decline in physical health, increasing signs of confusion and inability to handle routine tasks, the JFS care manager can help you evaluate what kinds of support services your loved one needs to support their health, well-being, and quality of life.
If your family members can’t agree on how to care for your loved one, the JFS care manager can facilitate communication and build consensus among family members.
If you are uncomfortable leaving your loved one at home alone, the JFS care manager will help you explore options to ensure their safety, security, and well-being.
If you live out of town, the JFS care manager will be your eyes and ears, ensuring that you know when there is a change in condition.
If you are worried that your loved one is at risk for fraud or is being taken advantage of, the JFS care manager will serve as their advocate with health, homecare, legal, and financial professionals.
