Services for Older Adults
and Their Families
JFS services for older adults are designed to promote and enhance
the health, well-being, social connections, and independence of
older adults in Orange County.
Care Management: JFS care managers coordinate and manage the care of older adults with complex care issues who want to continue living independently in their own homes. Care managers act as a surrogate for family members, many of whom live out of the area.
When do I need a care manager?
How does care management work?
In-Home Assessment and Care Plan: We will conduct a personal in-home evaluation of all the areas in which you or your loved one may need assistance. A care plan is then developed that serves as a roadmap for the future – giving guidance, reassurance, and recommendations for how to proceed in every area of concern.
Advice, Information and Consultation: Our eldercare specialists provide advice and consultation on various issues of concern to older adults and their adult children.
Family Meeting Facilitation: A JFS facilitator helps to lead family meetings in which family members discuss and come to agreement on critical care issues, relationship issues and division of responsibilities.
Counseling and Psychotherapy: JFS’ Geriatric Counselor provides sliding scale counseling sessions to address depression, anxiety, bereavement, managing change, and other issues.
“You and Your Aging Relative:” This monthly group in Irvine is for adults who are experiencing the pressures and responsibilities of caring for an aging parent or relative.
Volunteer Services: Trained volunteers provide weekly home visitation, reassurance calls or hospital visitation.
Community Rabbi: The JFS Community Rabbi provides pastoral visits to people who are ill or recovering from illness in the home, hospital or residential facility.
Bereavement Services: JFS offers both individual counseling and group support for people who have recently lost a loved one.
Holocaust Survivor Services: Care management, advocacy, in-home support services and social programming for elderly at-risk Holocaust survivors.
