Our care managers help coordinate and manage the care of older adults with complex care issues who want to continue living independently in their own homes. Typically, adult children call us when they are no longer able to juggle their caregiving responsibilities with the other aspects of their lives. Many live out of town and cannot provide the ongoing monitoring their loved one may require.
Care managers serve as the eyes and ears of family members, providing ongoing monitoring of the older adult and notifying family members when any change of condition occurs. First, the care manager comes into the home to do a thorough assessment. Then a care plan is developed, with recommendations about every aspect of care. After that, the care manager will coordinate all aspects of senior care, including providing ongoing advisement and intervention on homecare arrangements, financial, insurance, and medical issues; advising on transportation arrangements; providing crisis intervention; and serving as a liaison with medical professionals. Individual Consultation Sessions
In some cases, you may only need or want individual consultation sessions, rather than ongoing care management. Consultation sessions are available both in your home or in JFS offices in Irvine or Laguna Woods. Please call Cally Clein for information on service fees.
